Handy Help LLC delivers services from a foundation of hope and dignity.  

We believe that everyone deserves to live with dignity regardless of their past.  Our multi-faceted approach to services helps our community members make lasting change.     

 

The services for these individuals are provided through different waivers such as Community Access for Disability Inclusion (CADI) Waiver, Brain Injury (BI)  Waiver, Developmental Disability (DD) Waiver, Community Alternative Care (CAC), etc.    

 

INDIVIDUALIZED HOME SUPPORTS (IHS)

Handy Help LLC provides a wide spectrum of IHS services to support people with disabilities live in their own home or family home. 

There are three types of Individualized Home Supports:

  • Individualized Home Supports without training, or previously known as Personal Supports.  This service option is provided to adults and children when they need support in at least one of the community living services categories.
  • IHS with training, or previously known as Independent Living Skills. This service option is provided to adults and children when they need support and training in at least one of the community living categories.
  • IHS with Family Training, or previously known as In-Home Family Supports. This service option is designed for people who live with their family to providing training to the person and to their family members.

 

Corporate Foster Care or Community Residential Support (CRS):

Community Residential Services (CRS) or sometimes called Adult Foster Care, a housing option, for a person who prefers to reside in a residence with trained shift staff  available 24/7 delivering services.  Community residential services are individualized and based on the person’s unique needs as identified in the person’s support plan.

Emergency Assistance

Our 24 Hour Emergency Assistance Team is available around the clock to help provide on-call counseling, on-call problem solving and/or immediate response for assistance at the person’s home due to a health or personal emergency that is not related to an injury or an immediate danger.  Please know that this service does not replace the use of 911 when necessary.

HOME-MAKING SERVICE

Homemaker:  Services that help a person manage general cleaning and household activities.

Homemaker home management providers deliver homemaker cleaning services and, while onsite, provide incidental assistance with home management activities as needed. However, homemaker cleaning must be the primary service provided.

Home management activities may include assistance with:

  • Arranging for transportation.
  • Preparing meals.
  • Shopping for food, clothing and household supplies.
  • Performing simple household repairs.

Homemaker home management also includes monitoring the person’s well-being while in the home, including home safety.

Homemaker assistance with ADLs

providers deliver homemaker cleaning services and, while onsite, provides incidental assistance with ADLs as needed per the person's person- centered plan including but not limited to:

  • Ambulating.
  • Bathing.
  • Dressing.
  • Eating.
  • Grooming.
  • Toileting
  • monitoring the person's well-being while in the home including home safety.

TRANSITIONAL SERVICES

Transitional Services are provided through a waiver to fund goods, deposits and moving expenses necessary and reasonable for a person to move from an eligible setting to a home they own or rent.

A person can use transitional services to move to their own home from the following eligible settings:

  • Adult foster care home (corporate and family)
  • Hospital
  • Institution for mental diseases (IMD)
  • Intensive rehabilitation treatment or rule 36 setting
  • Intermediate care facility for persons with developmental disabilities (ICF/DD)
  • Nursing facility
  • Unlicensed setting (e.g., person’s own home, family home or integrated community supports setting)

NIGHT SUPERVISION

Definition: Overnight assistance and supervision provided by a trained staff in the person's own home based on the person's care plan.  

Night supervision services include an asleep or awake staff in the person’s own home performing activities including but not limited to the following areas: 

  • Implementation of the person’s positive support programming and transition plans.
  • Reinforcement of skill development supports (e.g., individualized home supports).
  • Assistance with activities of daily living (ADLs).
  • Assistance with instrumental activities of daily living (IADLs).

Chore Services

Definition: Assistance provided to a person or their primary caregiver to help maintain a clean, sanitary and safe environment.

 Chore services include, but are not limited to the following activities:

  • Rent a dumpster or dispose of trash.
  • Exterminate and provide pest control, limited to a reasonable number of treatments required to alleviate the pest problem.
  • Perform general indoor and outdoor home maintenance (e.g., lawn care, snow removal.)
  • Deliver groceries when the delivered products meet the majority of the person's total grocery needs for at least seven days.
  • Perform heavy household chores (e.g., secure loose rugs, wash floors, windows and walls).
  • Move or remove large household items and heavy appliances to provide safe access to and exit from the home.
  • Pack the person's belongings.
  • Rearrange or secure household items to prevent injuries or falls.

MA-Waiver Programs

Did you know that Minnesota’s MA-Waiver programs provide services that help people with disabilities live in the community rather than in an institution (MN.DB101.org)?  The MA-Waiver provides supports to an individual to live where they want in the community.  If you or someone you know is interested in the MA-Waiver programs, please contact their county of residence and ask about a MnCHOICES assessment for services.
