Individualized Home Supports (IHS)
The goal of Individualized Home Supports (IHS) is to provide services and supports tailoring to the individual’s need while living in their own home or family home.
There are three types of Individualized Home Supports:
- Individualized Home Supports without training, or previously known as Personal Supports. This service option is provide to adults and children when they need support in at least one of the community living services categories.
- IHS with training, or previously known as Independent Living Skills. This service option is provided to adults and children when they need support and training in at least one of the community living categories.
- IHS with Family Training, or previously known as In-Home Family Supports. This service option is designed for people who live with their family to providing training to the person and their family members.
All IHS types can be provided in the persons home, family home or in community spaces shared by the public and either in person or remotely.
Our staff members meet with the person receiving services, family, case manager, and other interdisciplinary team members as stated on the person’s Community Services amd Supports Plan (CSS) to ensure expectations and goal objectives are met. We always involve the individual, their family, and their interdisciplinary team in the development of the person’s CSSP because they know the person’s needs and wants.
IHS with Housing Supports have satellite offices throughout Ramsey County. Our small, skilled teams with expertise in the complex issues that are barriers to housing stability for this population.
We collaborate with landlords to increase housing options for program participants including studios or one bedroom apartment or shared living in a 2-bedroom or more. An example of IHS with training in the satellite office including support and training that addresses the person’s medical, behavioral health needs, medication management, home management skills, etc. using a person-centered and motivational interviewing approach that includes wrap-around services and promotes harm reduction and stability-based recovery.

Emergency Assistance
Our 24 Hour Emergency Assistance Team is available around the clock to help provide on-call counseling, on-call problem solving and/or immediate response for assistance at the person’s home due to a health or personal emergency that is not related to an injury or an immediate danger. Please know that his service does not replace the use of 911 when necessary.
