245D: Home Community Based Service
We offer individualized supports and services to adults with disabilities who are eligible for a waiver (CADI, IDD, BI, CAC, EW) program in the state of Minnesota. Handy Help is committed to person-centered practices and we consider each person as an expert in their own life. Our Home Community Based Services (HCBS) are person-centered, creative, flexible and meaningful. We partner with the individual, his/her family and significant people in their lives to identify meaningful life goals – physical, mental, emotional, spiritual, social, interpersonal, vocational and educational – and we teach them the skills to achieve them.
The HCBS offers people opportunities to live in the community of their choice where supports can be matched to a person’s unique needs and specifications. We provide wraparound services using independent living skills supports, homemaking supports, 24-hour emergency assistance, therapy, social skills groups, recreational activities, transportation, medication management, and care coordination; all involved are dedicated to assisting our clients to live as independently as possible.
Handy Help has partnerships with landlords throughout the Minneapolis/St. Paul area. These landlords provide the following benefits to our clients:
- Waive housing application fees.
- Work with tenants who have criminal backgrounds, history of unlawful detainers, or financial hardships that may have made acquiring housing a barrier in the past.
- Work collaboratively with Handy Help to assist tenants to maintain housing and explore ways to avoid late rent payment.

Individual Apartment
A person resides in his/her own apartment or where the person chooses to live and direct one’s own services and supports.
Community Living Setting
The individual chooses a rental living arrangement (apartment, duplex, condo, town home, renting a room), through Handy Help in which Handy Help provides wraparound services & supports to assist the individual to live independently. The lease or rental agreement is with Handy Help for up to 24 months. The goal with this option is to transition the lease or rental agreement to the individual. This is a great option for individuals with no credit or bad credit.

Supportive Housing with limited hours on-site staffing and no overnight staff
The apartment building is with a Handy Help landlord. Handy Help offers and coordinates supports for multiple individuals who each reside in their own apartment within the same complex. There is no satellite office in the apartment building and support staff is NOT located on site. Services and supports are based on the person’s coordinated service and support plan. Communication will include phone consultation and face-to-face check-ins.
Supportive Housing with on-site staffing 24/7
The apartment building is with a Handy Help landlord or is a Handy Help owned property. Handy Help coordinates supports for multiple individuals who each reside in their own apartment within the same complex. Handy Help has a satellite office within the same apartment complex and support staff is located on site from 8:00 a.m. to 10 p.m. or 24/7 as needed. Phone consultation in addition to direct face to face staff support is available to individuals.
Shared Housing with limited hours on-site staffing, no overnight
The house, apartment, or condo building is with a Handy Help landlord. Handy Help offers and coordinates supports for multiple individuals who reside in the shared house. This is a great option for people who enjoy the company of their roommate(s) or other people and have decreased rental costs. Services and supports are based on the person’s coordinated service and support plan. Communications include phone consultations and face-to-face check-ins.
