Disability Case Management

Case Management services play an integral role in assisting individuals to navigate complex public systems.  Services including but not limited to identifying, gaining, and coordinating access to needed behavioral health care, benefits & entitlements, social/housing/vocational services, and other supports to help people thrive in the community.

Handy Help Waiver Targeted Case Management provides culturally safe and trauma-informed services and supports to help each person served gain access to needed medical, social, educational, vocational, financial, housing, and other supports to help people thrive in the community.  We believe that services should be all about you. We tailor our services to fit your unique needs, choices, and abilities. Our staff is committed to take care of the administrative tasks, so you can focus on living your best life in the community of your choice.
