Case Management services play an integral role in assisting individuals to navigate complex public systems.  Services including but not limited to identifying, gaining, and coordinating access to needed behavioral health care, benefits & entitlements, social/housing/vocational services, and other supports to help people thrive in the community.

Handy Help Mental Health Targeted Case Management provides culturally safe and trauma-informed mental health targeted case management (TCM) services to help individual with a serious and persistent mental illness (SPMI) many whom have co-occurring disorders (SUD) and significant physical health needs gain access to needed medical, social, educational, vocational, financial, housing, and other supports to help people thrive in the community.

We are committed and dedicated to providing high quality care.  Our multi-cultural team are trained to actively apply recovery-based principals of care and provide care that is culturally and linguistically responsive to the diverse needs of the individuals of the community we served.

Case managers work to build relationships that foster hope, courage and resilience by helping the individual harness their strengths and motivates the individual to develop a roadmap that supports their journey to recovery.

Identifying what is important TO and Important For the person; identifying the person’s strengths, dreams/aspirations; ensuring housing stability; access to psychiatric/therapeutic, medical/dental and healthcare needs; crisis planning; coordination and monitoring of services; support to overcome daily life challenges and more….
